GP Management Plans for Chronic Diseases (CDs) are dedicated to supporting people struggling with chronic conditions by offering them a structured plan under medical guidance.

These plans are tailored to each individual patient in order to help monitor their health and to guide them to proactively managing their own health. This process ensures people understand their CD and are able to manage it in order to live a normal life despite their illness.

It is best to book a long first appointment to discuss a GP Management plan so that we can explain to you the steps for preparing a plan and register your agreement to go ahead with its preparation. You will then receive a copy of the plan and another copy will be added to your medical records.

A GP Management Plan (GPMP) contains:

– an assessment of your health problems, relevant issues, your healthcare needs;
– health goals you will agree on for this plan;
– the steps and actions to take;
– the medical services and treatments that will be required or most likely needed;
– how the medical services can be arranged for your convenience in a realistic way to achieve the goals.

The most important part is to regularly review the progress of the GPMP for ongoing management and tracking progress towards the goals you set. Our practice nurses can provide support and monitoring between visits to your GP.

A chronic medical condition is one that has been (or is likely to be) present for six months or longer, for example, asthma, cancer, heart disease, diabetes, arthritis and stroke.

There is no list of eligible conditions. However, these items are designed for patients who require a structured approach and to enable GPs to plan and coordinate the care of patients with complex conditions. This means patients who require ongoing care from a multidisciplinary care team. Your GP will determine whether a plan is appropriate for you.

If there are multiple chronic conditions you are dealing with, a single GPMP will cover your health needs as your GP will take all into account. There is also no age restriction to be eligible for a GPMP.
